We may bill your insurance carrier directly for covered services. You are responsible for any co-pays, deductibles, or non-covered services.
If you purchase equipment (such as CPAP devices or supplies), payment terms will be provided at the time of purchase.
You agree to provide accurate insurance and billing information and remain responsible for all charges not covered by insurance.

5. Durable Medical Equipment (DME)

Any DME provided (including CPAP machines and supplies) must be used according to the instructions provided.
You are responsible for proper use, care, and timely replacement of equipment.
We are not liable for misuse of equipment or failure to follow prescribed treatment.

6. Communications (SMS and Email)

By providing your phone number and/or email address, you consent to receive communications from us regarding appointments, billing, equipment updates, and other health-related matters.
Standard message and data rates may apply. You may opt out of SMS by replying “STOP” and opt out of email by contacting our office.

7. Intellectual Property
